The Internal Revenue Service issued $4 billion in fraudulent tax refunds last year to people using stolen identities, with some of the money going to addresses in Bulgaria, Lithuania and Ireland, according to a Treasury report released Thursday. The IRS sent a total of 655 tax refunds to a single address in Lithuania, and 343 refunds went to a lone address in Shanghai. In the U.S., more fraudulent returns went to Miami than any other city. Other top destinations were Chicago, Detroit, Atlanta and Houston.

An audit of the Orange County Crime Lab at the beginning of October uncovered the mistakes, which affect about 200 DUI cases but will only clear the names of about 20 people, said Bruce Houlihan, the lab’s director. The family of a 3-year-old killed in a northern Missouri house fire early Oct. 31 says it is outraged after police used a stun gun to subdue the boy's stepfather as he tried to run back into the burning house to rescue the boy. The U.S. Department of Health and Human Services is awarding more than $5.3 million in Affordable Care Act funding to support new health care centers in Georgia. In a release Thursday, department Secretary Kathleen Sebelius said the funding will help expand the delivery of health care services in the state, which is expected to impact more than 41,800 Georgians. Federal safety regulators on Thursday proposed major changes in workplace reporting rules that would require large companies to file injury and illness reports electronically so they can be posted online and made available to the public.
