    Searching for a vaccine exemption form for my wife to attempt to get her cardiologist to sign. She works in health care and her company doesn't have a form available yet, they say. She's had cardiac issues since a few years ago. From what I've been able to find so far the State of Michigan has medical exemption forms for children in schools, but nothing for adults.
