So, today my DH got his 2nd Pfizer shot.

He showed me he got a card for it.

I took a close look, and it's not just a card from his employer that administered the shots.

It's a federal-level card issued by Dept. of Health & Human Services (HHS).

My CDC card has my first and last name, my date of birth, labels off both packages of Moderna vaccine, and the dates and initials of the persons who gave me my shots and initials of the place where given that match the vaccine package labels.

One label reads:

Apparently you are so old that you don’t remember that most major vaccinations have come with record cards. Polio, mumps, measles, etc - all of them came with record cards to be placed in your ‘shot records.’ In fact, children have not been allowed to attend school since before the 1970s without proof that they have been vaccinated to whatever degree the school or state requires. Proof of polio vaccination started to be required in the 50s or 60s, IIRC. Even flu shots can come with notes to be placed in your records.

The reason it is that odd size is because it’s a traditional shot card size, originally intended (back before WW2) to be used with an index card file system in your doctor’s office.
