Keith Matthews has very narrow, steeply sloping shoulders in the picture, The “blackface guy” does not. May be the same pants; but, not the same guy IMHO.

Even in the 80s, even for “fun,” this was a racist pic. So even if this is his hilarious pic of his best friend and date in racist costume, why put it on his yearbook page? Clearly he curated the page. No medical school will choose random photos of each student for their yearbook page. It might not be him but a) it doesn’t make sense and b) at first the guy apologized for posing for that photo.
