Well, the article says Hussey "refused to believe" the black woman. So it sounds like Hussey kept asking even after the black woman said she was British - asking once would have probably been fine, but refusing to accept the answer and continuing to ask what part of Africa she was from would be considered annoying and offensive by most people.

For people who watched season 5 of The Crown, Susan Hussey is the wife of Baron Marmaduke Hussey (the “Dukey” in the TV show) who was head honcho at the BBC in the 1990s.
