The Secretary of Health and Human Services will establish a guidelines for a "standard health plan" which can be offered by health insurance issuers.  In order for a health insurance plan to qualify as a standard health plan it must provide at least the minimum essential benefits defined in the bill and have an 85% medical loss ratio (meaning $0.85 out of every $1.00 collected on premium payments must be spent on medical expenses of the enrollees).
