I don't do PhotoShop, but the right side horizon looks "pixelated" in the supposedly undoctored photo.

Blown up in Photoshop, the so-called undoctored image has less detail than the doctored one. They need to publish the full image as it came from the camera, with all the metadata.

I can't see the importance of it all.

If we let this one slip by as "oh, that's not so bad" then the temptation will be greater to slip one in of greater importance. Better to make it abundantly clear that (1) fakes will be detected & publicized rapidly, and (2) anyone faking news photos should & will be publicly flogged & fired.

It's like punishing someone for "reckless endangerment" - nobody got hurt, but we're still gonna punish 'em for taking the risk.

I think the bloodstained looking area is just a pattern. The man holding her is Green Helmet man. Somehow, he managed to get his helmet and orange vest off by the time he reached the ambulance, unlike in this photo.
