Thrown within 100 feet of Bush on stage. Pin was pulled. Georgian securiy got it. Thats all from NBC news.

CNN reporting the grenade landed approximately 100 feet from the stage, hitting someone in the back of the head. I'd pick it up so that everyone could see it and say "Excuse me, could whoever is throwing grenades please wrap them in safety foam first?".
You can never be to careful, since there might be kids about and bruises are unsightly.

I imagine that a preview of the video tapes of the occasion will show someone throwing something and someone getting struck in the head. Perhaps we'll be able to identify the perp that way.
Perhaps the video tapes will show the local security folks rushing the device and 'walking away' with it...
