A gay men STD screening may require more than a basic urine test. Some sexually transmitted infections (STIs) can hide in the throat or rectum. You could test negative on a standard screen while an infection remains untreated.
Urine testing can detect some infections, but it does not always check the throat or rectum.
Sexual activity can expose different areas to STIs. Gonorrhea and chlamydia can affect the throat or rectum.

Oddly enough, I have no concerns about any sort of STD. Huh. Imagine that.
Don’t bother changing your behavior; taxpayers will subsidize expensive pharmaceuticals to keep you in the game!

I can know with 100% certainty that I have no STI without getting tested.

Too much information. The “Gays” know the risks.
David Horowitz described the insanity of the 1980s in NYC and SF in his book “Radical Son”; it was a weird combination of defiance, denial, enthusiasm (to contract AIDS) and also simultaneously blame Ronald Reagan for not funding a vaccine, for the virus that was a myth, yet purposefully trying to contract, etc etc. Crazy doesn’t really begin to cover it. Health authorities have broad authority to protect public health, but... not in San Francisco, apparently. It’s all very tragic and sad and the rest of it.
From what I hear you can get STIs from toilet seats!
Hence the problem with “undetectable.”
They would always run those commercials

Maybe they should try armpits.
They simply assume they can take a pill to clear up any STDs. Many younger gays think the same when it comes to HIV/AIDS. Why worry? You just need to get on medication.
