Stronger quadriceps muscles, relative to the hamstrings, may lower the risk of total knee replacement, according to research. Researchers said the findings could inform strength-training programs for people with advanced arthritis in the knee.
"The two muscle groups act as counter forces, and the balance between them enables a wide range of activities while protecting the knee joint," said Upasana Upadhyay Bharadwaj, M.D. "An imbalance, in addition to other factors, leads to a change in the biomechanics resulting in the progression of osteoarthritis."
Dr. Upadhyay Bharadwaj and colleagues evaluated thigh muscle volume in 134 participants from the Osteoarthritis Initiative. They compared 67 patients who underwent total knee replacement of a single knee with 67 control participants who had not undergone knee replacement. The cases and controls were matched for variables.
The researchers obtained 3T MRI of the thigh at the time of surgery. They also evaluated MRI findings from two years and four years before the surgery. They used a previously trained deep-learning model to segment and compute volumes of the muscles of the thigh.
Comparing patients who had total knee replacement with the control group, a higher ratio of quadriceps to hamstring volume was significantly associated with lower odds of total knee replacement. Higher volumes of hamstrings and gracilis, a long, thin muscle on the inside of the thigh, were also linked with lower odds of total knee replacement.
"Our study shows that in addition to strong muscles individually, larger extensor muscle groups—relative to hamstring muscle groups—are significantly associated with lower odds of total knee replacement surgery in two to four years," Dr. Upadhyay Bharadwaj said.
The study findings have implications for both the interpretation of imaging exams and clinical management. The results suggest that training programs that strengthen the quadriceps in relation to the hamstrings may be beneficial.

Soccer trainers have known this for years relative to minimizing knee injuries.
"I'll NEVER replace my knees!"
Short sided...it is the other way around. The hamstrings are notoriously, weak in many people, which causes imbalances between each leg and causes issues which posture and tightness of the IT band, which causes improper Gait and knee tracking (the knee cap begins to point left or right, slightly. This coupled with low vitamin D status for too long, weak mud-back muscles, placing a wallet in your back pocket on a daily basis, crossing your legs, too long and pointing your knee towards the center of your car or door, while driving in traffic, is what raises your risk for a knee replacement.
Ugly legs.
Pretty girl. Gross legs
I’m 84. I make it a point to climb at least 10 flights of stairs everyday.
Yes, but if she was your woman, you'd never have to pay for a knee transplant.
Stairs, strengthen the hamstrings, so you are on the right track!

well I’m just 70 but I climb the stairs either upstairs to our bedroom or down to the bottom basement floor....probably several times a day....I have very bad knees..
Bkmk
Strengthen your mid-back muscles and ankles and your knee pain should improve. Also, alternate which leg you lead with when you climb the stairs.
I’m still amazed by the fact that despite both my hips having imploded due to “overwork” (my word) I’ve never had a lick of trouble with either of my knees. One would thing that the same stresses that destroyed my hips would damage my knees...but they didn’t.
I don’t think so
Knee replacement is about cartilage destruction and bone on bone pain.
Strengthening quads make for easier knee replacements
What causes your hips to go, is low vitamin D status, improper Gait and weak ankles.
My ex-gf had terrific thighs, could crush a walnut if placed between them.
True, but those legs would not be on my woman. I have the good fortune of having a pretty wife with some nice gams. 😁
