Pretty sure it’s bad to be fat, no matter what culture you are in.
There is no such thing as being fat and healthy. Doesn’t mean everyone should be anorexic either - but if you are carrying around the equivalent of a small child in excess weight, you are unhealthy. Period.
Want to be fat? be fat - I have the same struggles as everyone else - but just admit you are fat, and unhealthy - don’t try to convince everyone is it’s just a perception problem - it’s not.

The doctor in the OP is part of the problem. You don’t have to “focus” on it, but it’s important to bring it up. And kids/young people should really be encouraged to tackle it early since it’s much easier to make changes then.
I lost a significant amount of weight once between appts due to a lifestyle change. I actually had to bring it up to the doc who said that she had noticed it, but that patients often don’t like her to bring up the topic of weight. Um, what if I had lost that weight without changing anything? Seems like a highly medically relevant thing for her to at least to ask about.

Well I guess that means America was a fair and equitable place, 50 years ago and prior. Everyone was pretty much height weight balanced back then.
There's a will and there's a won't. Take your pick and then live with the results. But don't lie to yourself.
Light but consistent daily exercise of lifting weights and walking/sprinting.
Eating reasonably. 2000ish calories a day. Take it easy on the carbs/starches. Avoid sugar.
Easy peasy.
While being fat is not healthy, neither does being thin by default mean you are healthy.
Someone who is naturally thin can have a very unhealthy diet and lifestyle because they don’t see the need to eat well or exercise.
Common sense and non-sensationalized clarity of insight is out of vogue these days. Being too fat or too thin is not healthy or balanced. Unless some medical issue is prevalent the biggest issue is self discipline or lack thereof. Eat less or more and whatever the results the individual owns it. Attempting to justify bad behaviors and habits or blaming others is simply a loser mentality. Plenty of that these days.

The correct word for this is ai̱día, which is Greek for 'disgust'.
Fat-phobia, homophobia, islamaphobia, ad nauseam. It's not fear, media lackeys, it's disgust. A plain, simple and natural reaction to something that is to be avoided, and certainly not worthy of praise.

As a volunteer firefighter we sometimes get calls to do EMS assist which is usually helping a crew get a biggie in the ambulance.
About 4 weeks ago we had one, it took 5 of us about 20 minutes to move the patient approx. 6 feet from the bathroom to the kitchen. Patient had to be about 550.
You get skinny the same way you got fat: one fork-full at a time.
I think you will find a lot of people getting thinner because food is getting way too expensive. Gas as well so maybe people will walk more.

Yet, the body’s processes are easily altered and damaged by a multitude of endless causes. Tiny hormonal deficiencies, medicines, diseases, to name a few.
It is not the exception. Hormones, disease, medicines, can and do effects weight.
Yes, there are many people who overeat and under exercise.
But most overweight people wouldn’t be if they had a choice.
Spoiler alert, lots of drugs and other environmental items are damaging the natural processes of the human body.
