Those blank card images are all over the net. Just put 'COVID card' or 'COVID CDC card' or similar in your favorite search engine and select 'images'.

How do you print so that it is on front and back? I get a mismatch...

Forging an official document is a crime. This would be forgery due to the CDC logo. There could be penalties or consequences. I do not recommend it.

That being said, owning counterfeit Raybans is a crime as well, whereas owning a pair of commonly sold Roybans is not. The enterprising counterfeiters change small unnoticeable parts of the logo to avoid copyright infringement. Sometimes even hide “For entertainment use only” in the illegible fine print.

I am not a lawyer and suggest nothing whatsoever to anyone by this observation. Not even to the photoshop folks here.

Thank you. There seems to be an issue printing to get the front and the back of the card on the same piece of paper...if that makes sense.
