The Official Preliminary Report is out, and confirms the fuel cutoff switch directly after takeoff: both switches found in in the off positions.

Not quite correct. According to the official Preliminary Report, the switches were put into the cut-off position but were then put back into the run position - and both were found in the run position. It appears, unfortunately, they were put into the cut-off position at just about the worse possible moment - somewhere between V1 and VR on the take off roll - which left not enough time to restart the engines after take off.

No, that’s not how the system works.
There seems to be some history of those switches being installed with the lockouts disabled. I'm uncertain if the problem lies with the components of the installers. Either way, that should be a punch-list item to be checked before the jet's initial flight.
A guy name Captain Steeeve has a YouTube about it. He also has a doctorate in counseling. He ends his analysis with a talk about the problems of pilot suicide

A friend of mine, had his late 1950’s Beech Bonanza completely restored. Looked and ran great!
Within days, he landed . . . without lowering the landing gear.
Friend either forgot . . . or imagined that he had lowered the gear.
I’d read that there is a fire prevention scenario that will activate the cutoffs automatically. The idea that there were cutoffs lurking in the system scared the hell out of me.
These so called switches don’t work like regular SPST switches. The must be pulled up, rotated, then pulled down. This was a suicide mission.
