The Blackhawk helicopter was flying too high, by a lot. It was far above the 200 foot limit. That's not really too complicated to understand, is it???
Why was the Helo too high? Both pilots would have known how high they were.
Then to learn the control tower was understaffed?
The whole incident was preventable.
Don’t forget that only a few weeks ago that an even deadlier accident was barely avoided at Los Angeles and only saved by a controller who slipped through the cracks as he was not DEI, although at least one DEI was in the plane that nearly caused the accident.
The pilot definitely doesn’t fit the profile of a terrorist.

Sounds like they had the Helicopter traffic controller go home early. He may have picked up on that.

* All FAA has been understaffed.
* FAA cut back hiring ex-military ATC with experience to reduce white males. Instead of asking about aircraft and ATC experience, the interview asked about sports played in high school!
* DCA ATC has been understaffed a long while.
* Congress has authorized more landing “slots” at DCA (one of three airports in the USA that use the “slot” system) increasing traffic at DCA.
* Normally there are separate people handling helicopter traffic at DCA and fixed wing aircraft at DCA. The supervisor let the helicopter person leave early.
* The ATC woman did not provide azimuth and distance to the CRJ, jut a vague “Do you see the CRJ” to the helo pilot.
Sounds like overwork to me. That and “all the holes in the Swiss cheese lined up.”
1) Helo was too high so crew was at fault.
2) ATC didn’t warn helo it was climbing above the 200ft threshold and didn’t provide azimuth or potentially enough info to identify the aircraft leaving the helo open to misidentifying plane in busy area - ATC at fault (understaffing>)
What should have happened imho, helo should have been ordered to stop and return to proper altitude - basically a major warning of potential collision.
