My theory has always been that the first use of Islamic nukes will be against other Muslims. Some in the West see Islam as all alike. But a finer grain picture is they more closely resemble the schism in the Catholic/Protestant world in the Middle Ages. There were long wars over who would rule Christendom. Think of the most recent Hamas attack on Israel. While Iran supported the attack, they won’t get involved because they’ve repeated that Hamas are apostates and should die. Whether they should die before or after the Great Satan is up for grabs.
Thing is that the basic tenants of Christianity allowed them to eventually work things out and coexist. There are no allowances for compromise or coexistence in Islam. An Israeli Prime Minister once said, “If there were no Jews to focus hate on, Islam would be forced to invent us.”

I'd suggest a wee bit earlier - when the EO split away from Rome.
Especially the Shia/Sunni divide.
There had been suicidal attacks on each other mosques, clerics advocating killing of each other, etc.
There is also a radical/moderate divide (most Muslims are actually moderates, but scarred to death of the radicals!)
and the national and regional divides.
