Well, I guess when a guy invests so much emotional energy into recruiting others to go along with his fantasy, learning that they don't can be quite a shock.

He clearly needs therapy. Unfortunately, the therapy he is receiving probably avoids his root issues. His problem is not that he believes he is a woman (he doesn't) but that he is unhappy being who he is and wishes he were someone else.
