Variants can arise any time a virus replicates. When someone is infected their body is turned into a virus making factory. Viruses do not reproduce themselves - the cells of the host they infect does - viruses inject their genetics into cells, which in turn programs the cells to produce viruses.

An infected person will produce many billions of viruses. It only takes one of the billion to have a mutation and then escape to infect another host to produce a variant.
