Abstract BACKGROUND:

The Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) first broke out in Wuhan (China) and subsequently spread worldwide. Chloroquine has been sporadically used in treating SARS-CoV-2 infection. Hydroxychloroquine shares the same mechanism of action as chloroquine, but its more tolerable safety profile makes it the preferred drug to treat malaria and autoimmune conditions.

We propose that the immunomodulatory effect of hydroxychloroquine also may be useful in controlling the cytokine storm that occurs late-phase in critically ill SARS-CoV-2 infected patients. Currently, there is no evidence to support the use of hydroxychloroquine in SARS-CoV-2 infection.

METHODS: The pharmacological activity of chloroquine and hydroxychloroquine was tested using SARS-CoV-2 infected Vero cells. Physiologically-based pharmacokinetic models (PBPK) were implemented for both drugs separately by integrating their in vitro data. Using the PBPK models, hydroxychloroquine concentrations in lung fluid were simulated under 5 different dosing regimens to explore the most effective regimen whilst considering the drug's safety profile.

RESULTS: Hydroxychloroquine (EC50=0.72 μM) was found to be more potent than chloroquine (EC50=5.47 μM) in vitro. Based on PBPK models results, a loading dose of 400 mg twice daily of hydroxychloroquine sulfate given orally, followed by a maintenance dose of 200 mg given twice daily for 4 days is recommended for SARS-CoV-2 infection, as it reached three times the potency of chloroquine phosphate when given 500 mg twice daily 5 days in advance.

CONCLUSIONS: Hydroxychloroquine was found to be more potent than chloroquine to inhibit SARS-CoV-2 in vitro.

There are currently several clinical trials using a variety of well known drugs to combat the coronavirus. One drug is the one used since World War II for treating and suppressing malaria. The drug is chloroquine.

This article shows that the sister drug hydroxychloroquine is even more potent in killing the virus off in vitro (in the test tube to not in the body.) it is also less toxic with very few side effects.

Hydroxychloroquine has been used for decades for travelers going to malaria infested countries. I am sure there are many readers here that have taken the drug or chloroquine when they’ve been traveling on business to these countries.

I’m suggesting that you might want to consult your doctor to see if he or she might put you on hydroxychloroquine to reduce your risk of infection with the coronavirus. This drug was also used during the SARs epidemic. IN FACT, IT IS BEING USED FOR TREATIN G THOSE INFECTED WITH THE CORONAVIRUS AS WE SPEAK.

Since at the present time it is not FDA approved you may need to sign a waiver to hold your doctor harmless, even though there are very few side effects from the dosage suggested in this article. The whole purpose here is to bide some time to get us thru this period of exposure until a vaccine is available next year.

Because of my age and my potential exposure to the virus I decided to go ahead and start the drug. I’m taking 400 mg twice a day for day one then 200 mg twice a day for four days. Then I will take 400 mg twice a day every seven days after that.

Let me emphasize that I have no doctor patient relationship with any of you here I’m just simply recommending that you look into this drug as an adjunct to reducing chances of contracting the infection, and consult with your doctor especially if you are at risk or have been exposed to somebody with the virus.

I predict in the coming days it will be a recommended drug to use as a prophylactic.

Side effects are minimal. With prolonged used, ie several weeks, it may cause retinal damage.

Thank you sir for posting this.

I read about the drug and saw a video yesterday about it’s use. Something was said in it that stated that Zinc was necessary for it to work best with the Corona virus.

My thought is that patients at high risk for serious disease should get this if they have been exposed or at least if they are in early stages, before they go critical. An investment of resources.

This looks like a relatively easy drug to synthesize. Do we have the capacity to produce large quantities quickly?

chloroquine (similar to Hydroxychloroquine) was the drug mentioned in the article I had read.

I’ve bought some Quercetin but haven’t started taking it yet. Not sure if i’m going to take it as a possible preventative or take it if i begin to have any flu-like symptoms. I doubt it would be as effective as hydroxychloroquine (if it’s even effective at all), but i’m not in a high risk group. Theoretically, it would function similarly—getting zinc into the cells to block virus replication.

Coincidentally, I’ve also been taking turmeric/curcumin which i began using a couple of years ago to see if it would help with some elbow inflammation. Seems to. Poking around the internet tonight, i ran across studies that say it may also be anti-viral. Guess it probably won’t hurt with the coronavirus.

I think it is reasonable for the empirical use of hydroxycholoquine in the setting of known exposure to the virus especially in individuals with comorbidities.

I have started taking Quercetn and low dose zinc as a prophylaxis based upon data shown in this video. https://youtu.be/vE4_LsftNKM

Got a prescription for quinine and it is full.

I was using it for severe muscle cramping caused by my lyme disease. However, every since I started Lyme treatment, cramps are gone, thank God

Yes, zinc is needed.

When I started taking the quinine, the dr made sure that I was on a high potency zinc multi vitamin.

No, actually we don't. There are pharmaceutical manufacturing sites all over the world.

There’s a study coming out of 30 confirmed Coronavirus cases where they put half on the hydroxychloroquine and the other placebo. They claim all the treated cases had no virus detected, where as the untreated continued to shed the virus.

Hydroxychloroquine is very safe. Has virtually no side effects, esp. short term usage. Chloroquine is safe too, but has more side effects, like the one you list.

So did it shorten the illness? Reduce the hours of vent required? Did it actually help the patient or is this just to limit the spread from “recovered” patients?
