I encountered something similar at a baseball postseason invitational tournament for elementary through middle school teams. My son was on a 8-9yo team and the team was in the championship game. In the middle innings, the opposing team started frequent pinch runner substitution.

A couple of the parents in our sides bleacher questioned that one of the batters at the plate had been pulled out earlier for a pinch runner and was now back in the game with a different number. We all started watching a lot closer and saw this happen a second and third time. Lots of shouting to the plate umpire finally got his attention and he kicked a couple of players out the game for jersey swapping Didn't kick the coach out as well though. My son's team won the game.
