Did Hussein's Middle name appear on any state ballots?

Forgive me if this has been covered before. According to the West Virginia elections website it did appear in that state.

I'm curious if it's the only one.

In Illinois, there was no middle name mentioned, and Barack Obama had top billing on the ballot. Not only that, John McCain was listed in the THIRD spot (not second), under a no-name duo.

I’m in Chicago, I don’t recall the ballot order. It may alphabetical in some way?
I do know 0bama is first demorat to get over 60% of the POTUS vote in this state.

No middle names here in GA. Now, will it be used in the swearing in ceremony? Both Bushes and Clinton did - Ronald Reagan didn’t.

I find his first and last name as scary as his middle name. So I never got the middle name obsession.
It wasn't on in Pennsylvania, I checked for some strange reason.
