Daily Kos posts copy of Obama's birth certification

No word of how he got it-Team Barry isn't actually leaking to dKos, is it?-and naturally he misunderstands Jim Geraghty's point about Obama's place of birth, which would have been relevant not with respect to his "Americanness" but whether he was constitutionally qualified to run as a natural-born citizen. But here it is.

Update: Not that it matters for purposes of addressing Geraghty's concerns, but Flip notes that the posted document is a certification of birth, which is slightly different from a birth certificate.

How to Apply for Certified Copies of Vital Records

All applications requesting certified copies of birth, death, marriage, and divorce certificates must generally be made in writing (application forms may be downloaded from this site - see below). Requests may also be placed for birth and marriage certificates on a limited basis through the Internet (www.ehawaiigov.org/ohsm). Telephone, FAX, or e-mail requests are not accepted.
Fees for certified copies of birth, death, marriage, and divorce certificates are identical:

$10.00 for the first copy of each certificate, and
$4.00 for each additional copy of the same certificate ordered at the same time.

There is an additional fee for requests made through the Internet as follows:

$1.50 for a request of one (the first) copy of each certificate, if the requested certificate is found, or for the cost of conducting the search, if the requested certificate is not found, and
$0.25 for each additional copy of the same certificate ordered at the same time.

Fees may be paid by cash (except for applications made by postal mail), money order, certified check, or cashier’s check - make money order and checks payable to the State Department of Health.
Fees must be paid by a charge to a credit card for requests made through the Internet.
All fees for certified copies are payable in advance and nonrefundable. If no record is found after a search is conducted, the fees are retained to cover the cost of the search.
Once an order has been received and processed, a $10.00 fee will be charged for any request to make changes to the order.
Apply In Person

Walk-in service is available:

Days - Monday through Friday (excluding holidays)
Hours - 7:45 a.m. to 2:30 p.m.
Location - Room 103 (1st floor) of the Health Department building, 1250 Punchbowl Street (corner of Beretania and Punchbowl Streets)
When applying in person, the applicant must show a valid government-issued photo ID, such as a State ID, driver’s license, etc.

Certified copies are usually not issued on the day the application is made. Same-Day service may be provided upon presentation of written documentation establishing the need for urgency. Certified copies will normally be available for pick-up about 10 working days after the request is approved. The pick-up time may be extended for records that are very old, because the search to locate the record may take longer, or in the process of being filed, because the official record is still being created.

Application forms are available in the building’s lobby area and should be filled in prior to coming to the counter in Room 103.

Apply by Mail

Send mail-in applications to the following address:

State Department of Health
Office of Health Status Monitoring
Issuance/Vital Statistics Section
P.O. Box 3378
Honolulu, HI 96801

When applying by mail, the applicant must include a photocopy of his/her government-issued photo ID, such as a State ID, driver’s license, etc.

Certified copies will normally be sent out within 5-8 weeks after receipt and approval of the application. The return time may be extended for records that are very old, because the search to locate the record may take longer, or in the process of being filed, because the official record is still being created.

What Information You Should Be Prepared to Provide

An applicant/requestor must provide the information needed to 1) establish his/her direct and tangible interest in the record and 2) locate the desired record. This will normally include:

Applicant’s name, address, and telephone number(s);
Applicant’s relationship to the person named on the certificate;
Reason why you are requesting the certificate;
Full name(s) as listed on the certificate;
The certificate’s file number (if known);
Month, day, and year of the event; and
City or town and the island where the event occurred.
For birth certificates, also provide the full name of the father and the full maiden name of the mother.
If you are applying for a certificate on behalf of someone else, you must provide an original letter signed by that person authorizing the release of their certificate to you.
Letters of Verification

Letters of verification may be issued in lieu of certified copies (HRS §338-14.3). This document verifies the existence of a birth/death/marriage/divorce certificate on file with the Department of Health and any other information that the applicant provides to be verified relating to the vital event. (For example, that a certain named individual was born on a certain date at a certain place.) The verification process will not, however, disclose information about the vital event contained within the certificate that is unknown to and not provided by the applicant in the request.

Letters of verification are requested in similar fashion and using the same request forms as for certified copies.

The fee for a letter of verification is $5 per letter.

Application forms are available in a “fillable” Adobe Acrobat portable document format (PDF). The Adobe Acrobat Reader 7 (or later) is required for using the fillable forms feature.

WHY? Why would it ask for race at all?? MINE sure dont!

Mine does, issued by Illinois in the 1950's. My wife's does as well, from Ohio during the same decade. My daughter's doesn't, but her's was issued by Illinois in 1991. It wasn't at all uncommon for birth certificates to list "Race or Color", as that part of the document puts it, yeaws ago. That has changed over the last 20 or 30 years.

I'm calling bullshit on this one until proven otherwise. No name of attending physician, no name or location of medical facility, no height or weight of the referenced larvae, and the list goes on. I had to get a copy of my birth certificate from The People's Republic of Maryland a few years ago, and it included everything but a stool sample. That thing is akin to getting one of those credit card mailers that enclose a generic card with 'John Q. Public' embossed on it.

Look at the bottom of the form:

This copy serves as prima facie evidence of the fact of birth in any court proceeding.

To the right of that is says this about the form:

OHSM 1.1 (Rev. 11/01)LASER

This IS a computer printed form and probably a genuine, "CERTIFIED COPY OF A BIRTH RECORD".

This is not a "Buckhead Moment"

It may well have asked for race back in 1961. This probably varies from state to state. But I don't think they would have written "African" for race in 1961. This was even before "black" was used. If it was referring to his nationality, since there are plenty of people who are "African" who are not black, they would have written "Kenyan" rather than "African".

Also, notice that this has the date of "Jun - 6, 2007" backwards on it as though a freshly printed date from something else was accidentally transferred to it. It couldn't be another birth certificate or issue date because the font and abbreviation is wrong. If the office that does the birth certificates prints "August 4" it's not going to be printing "JUN - 6" for "June 6".

I see it as June 6th 2007 too.

His campaign probably had to provide this documentation to various states’ Department of Elections with other paperwork, to pass state level laws and regulations regarding candidacy filings.

This document isn't a birth certificate. It's a certification of birth, I'm looking at my certification of birth right now, and it doesnt show race...... but my birth certificate does.

Not necessarily true. My wife got one two weeks ago with one. Her original was lost and all we had was the certificate.

My guess is that the Obama campaign is trying to gin up the controversy over Obama’s Muslim heritage, not over his place of birth. Republicans can hardly hit Obama for his place of birth when McCain was not born in the US, either.

There were posts on FreeRepublic, speculating that Obama had two middle names, Hussein and Mohammed, and that Obama wouldn’t release his birth certificate because he didn’t want it known.

It appears that the Obama campaign is trying to marginalize the anti-Obama voters by labeling them as bigots (against both Blacks and Muslims) and nuts. Rush said that the attention on the Michelle Obama “Whitey” tape is the same thing, just an attempt to depict the conservatives as nuts, and offensive for making accusations against Michelle. It’s possible that the whole Whitey tape rumor was generated by the Obama campaign for this purpose, especially since Bob Beckel was the one who brought it out and gave it legs, on FOX News.

The word LASER on the lower left also indicates it is not original. The first working LASER wasn’t operational until 1960 and even then in a lab with no printing applications even dreamed of by 1961.

Absolutely correct. Anyone with half good skills could fake the document posted at crazyKos. Real birth certificates have notary seals or stamps of some type.

OKAY PEOPLE LET ME PUT THIS ALL TO REST:

BARRY WONT RELEASE HIS BIRTH CERTIFICATE BECAUSE IT LISTS HIS RACE ON IT. AND HIS RACE IS LISTED AS “WHITE”. IN THE 60’S AND 70’S HOSPITALS WOULD ALWAYS MAKE THE BABY’S RACE THE SAME RACE AS THE MOTHER.

According to Macranger at macsmind, he called Hawaii for verification on race being listed as African. He was told it would have been listed as Black or Negro, not African.
