Not necessarily true. My wife got one two weeks ago with one. Her original was lost and all we had was the certificate.

This certificate would not get you a passport because it is not certified or has no official notary. It would be declined.

Not necessarily true. My wife got one two weeks ago with one. Her original was lost and all we had was the certificate. Yes, but it had a signature and it had an appropriate stamp on it. This one has neither. There is no way it could get a passport. They would reject it in a nanosecond.
