A piece of dust or dirt on the lens. Dirt or dust on the lens NEVER shows up in an image. The surface of the lens cannot be focused. Those serve only to reduce the light reaching the film or sensor.
It is probably a dead pixel in the sensor or a dropout during transmission of the image.
