I would think it’s done at hospital surgically and then recipient gets it.

Okay, this guy is a freak. But the process is called Eye Enucleation. When I worked in Funeral Service, I learned about it, but didn't get certified. My dad donated his eyes.
And yes, it can be done by a Funeral Director, not an Ophthamologist.
