India's New Covid treatment guidelines drop ivermectin, zinc, vitamins, hydroxychloroquine: DGHS norms at variance with ICMR advisory

India on Monday issued a new clinical protocol for Covid treatment and dropped all commonly used treatments for mild patients of the infection, including antibiotics, hydroxychloroquine (HCQ), ivermectin and antiviral drug favipiravir.

No antibiotic for mild cases

WHO chief scientist Soumya Swaminathan today hailed the new evidence-based guidelines developed by the Director General of Health Services (DGHS) who said mildly ill Covid patients would be fine with just paracetamol, cough syrup and budesonide inhalation and need not be prescribed any antibiotics or major drugs at all.

This means the currently in use and much prescribed HCQ, favipiravir, ivermectin, azithromycin, doxycycline, zinc and vitamins are not required clinically and have no evidence of benefit.

The DGHS advice, however, is at variance with the ICMR advisory, which recommends both ivermectin and HCQ in mild cases. For asymptomatic patients, the new guidelines do not even recommend a blood test or any drugs and clarify “no investigation and no medications are needed at this stage”.

Mild patients would need investigation and paracetamol if symptoms of fever, cough, fatigue and other classical Covid signs persist or deteriorate.

The guidelines say no to Baba Ramdev’s Coronil and 2-DG drug for moderately ill patients and recommends monitoring of oxygen saturation, controlling comorbidities and giving steroids and anticoagulants as per medical advice.

The guidance lays down clear indications for the use of tocilizumab drug in severely ill patients and also tells doctors how and when to prescribe. It stresses the need to recruit a hospital infection committee to ensure prevention from fungal infections like mucormycosis and spells out a clear use and dosage of amphotericin B, which is in short supply currently.

The new advice says no to irrational and rampant use of chest CT scans and indicates when to use a chest X-ray. “Exercise extreme caution when ordering a chest CT scan. Patients must remain socially engaged and stay positive,” the guidance says.

Sounds like it. I was thinking they would go after India for manufacturing and exporting Ivermectin. Maybe this is step 1.

From the article, it says:

The next question of course is this — why paracetamol? What therapeutic good does it have and like any other demand they have, the same question applies — do you have any studies to show that it is effective?

if someone is taking aspirin pre-hospitalization it might mess with the dosage of anticoagulants the patient may need within hours of hospitalization? only thing I can think of

This stop all mild case treatment reminds me of when general antibiotic use was suddenly phased out in response to the exponential growth of antibiotic-resistant bacteria. This may be a similar step - slow the mutations. Otoh, India is swamped with patients and short on everything, so it may be a political out for doctors and hospitals to justify turning away all but the most critical patients.

Study shows hydroxychloroquine and zinc treatments increased coronavirus survival rate by almost three times

A new study shows that the controversial drug hydroxychloroquine touted by former President Donald Trump increased the survival rate of severely ill coronavirus patients.

The observational study, published by medRxiv, found that antimalarial drug hydroxychloroquine, along with zinc, could increase the coronavirus survival rate by as much as nearly 200% if distributed at higher doses to ventilated patients with a severe version of the illness.

“We found that when the cumulative doses of two drugs, HCQ and AZM, were above a certain level, patients had a survival rate 2.9 times the other patients,” the study’s conclusion states.

I was under the impression that ivermectin showed such promising results. The same goes for HCQ as well as zinc etc. Do the powers to be prefer to have people die instead? Did someone get paid off ? There appears to be something sinister about the whole thing...

No, here is the new treatment.

1. Tylenol

Sounds like a similar treatment the Jews in Auschwitz got when the Nazi's had Beethoven's Fifth Symphony played for them. I would think some social unrest might develop over this travesty.

Tylenol for those not familiar with other names.

All it does is help relieve fever and pain symptoms. No antiviral properties.

And if you take more than 4000 mg daily, it will chunk out your liver.
