Cholinesterase inhibitors - a pharmacological dementia therapy - often times affects the pupil size (miosis). It’s a very common side-effect to that class of medication.
Additionally, microvascular changes which can manifest themselves in the eye - in subconjunctival hemorrhages, for instance - can also be a contributing or indicating feature of dementia. IOW, eye condition is something that is closely measured in dementia/Alzheimer patients.

Some day someone is going to spill the beans on what happened behind the scenes when Biden’s eyeball filled up with blood in the middle of a townhall meeting. I think that was a big turning point in the efforts to fix the democratic primary.
