Overall, exercise equipment as a category can be particularly dangerous for children: 25,000 kids under the age of 10 are injured by exercise equipment each year, according to a 2014 study from the US National Institutes of Health. In 2019, the Consumer Product Safety Commission estimates U.S. emergency departments treated 2,000 treadmill-related injuries in children under 8.
Peloton (PTON) said in a statement to CNN Business that there are "no words to express the shock and sadness everyone at Peloton feels as a result of this terrible tragedy." The company won't be disclosing any additional information about the incident "out of respect for the family and privacy."
Foley said Peloton recommends its customers keep children and pets away from equipment at all times. Before a workout, they should ensure the area is completely clear — and once they're done, they should remove the treadmill's safety key (required to be inserted for the machine to work) and store it out of reach.
NIH similarly recommends positioning treadmills to face the doorway, or using a mirror to improve visibility, if children are present in the household.

Treadmills can be tricky if you haven’t quite figured out how to safely dismount them. The best way is to cut the power off and just keep walking, waiting until it comes to a full stop.
Many people don’t want to wait that long, so they’ll devise other ways to leave the machine. If one is not in coordination with that ever moving belt, you could easily find yourself thrown off the horse, so to speak.
3 yo boy was trapped under a Peloton Tread Plus, found by his father to be not breathing and pulseless - he was resuscitated and now has significant brain injury. He was found to have tread marks on his back matching the slats of the treadmill, neck injury, and petechiae on his face, presumably from occlusion of blood flow.
At one time I had a treadmill. It was in the basement Family Room, next to the bar. The cat liked to sit on the bar and watch me run.
One day, he decided to jump on my shoulders from the bar, then fell off, hit the treadmill belt and was flung into the wall behind us. WHOMP!
He only did that once. (He was OK; lived to a ripe old age.)
Sorry this family is going through this. Maybe Harry Reid can advise them on these types of accidents. ;)

My 6 year old granddaughter loves my treadmill and begs to use it whenever she’s over. I guess I’ll “forget” where I put the safety key next time she visits. Sigh.
