I've been messing with numbers again. Below is what I've determined to be our day by day case (whatever a case is), fatality ratio after offsetting 14 days from a case initial date until time of death using worldometers data. The large spike, I believe is the NY data dump. These numbers seem too large, but I've double checked everything.

Not that big of a sample, but look what it was near the beginning, and how much it has dropped! If this is accurate, the CFR (ratio) was/is much higher than we were led to believe.

I think it indicates improved treatment effectiveness along with the Coronavirus being far deadlier for some during the initial outbreak, probably for the elderly before stringent quarantines.

Even 4% is a far piece from population immunity. And this in a part of the country that some want to believe already *has* population immunity because of so much travel with China.

It does possibly indicate that circulating D3 levels may be ‘the great attenuator’ for this disease’s outcome.

yeah I didn’t realize there was this level of discussion, I haven’t been around texags much in a while.

I run them more wet than most people would likely prefer and once they move from the infectious to inflammatory stage I am very liberal with steroids and IL-6 inhibitors, almost everyone goes on anticoagulation unless you have a contraindication. My admissions come in incredibly ill now because nobody will come to hospital until they’re about to die so its hard for me to know what helps in the early infectious stage at this point. intubation is almost a death sentence, I will tolerate you tanking for a long time before I will intubate, prolonged RRs in the 40’s or hypoxia in the 60-70s for a few hours if you are not having arrhythmia issues. I’m praying for some type of legal immunity after this is over because it has truly been trial by fire the last few months.

I have a collection of maybe 250 of the worst CT scans I have ever seen. Imaging I would have believed was incompatible with life just a few months ago.

I’m getting much more pushback from the pharmacy with the actemra now. They want hep panels and quantiferon and HIV screens. We’ve come to the agreement that they’ll give it to me as long as I draw the labs first but we don’t have to wait for the results.

Soon I’ll run into a supply issue like everything over the last two months. Currently I am short on ventimasks, MDIs, phlebotomists and its almost impossible to get blood products. I am lucky there are a lot of residents from India and Cuba who are expert phlebotomists from their time as medical students in their country who will help me obtain labs as I’m a worthless bougie American trained doctor. A lot of people here will hate me for saying it but the residents in NYC should all be given medals and offered permanent residence, they are legitimate heros who could have gone back to their homes and been doctors but chose to stay.

I have commented on this on several threads. IMO it is not. Their PO2s are damn low. And PO2s correlate with the normal O2 / Hgb dissociation curves. I.E. no rogue Hgb species.

I hope everyone is okay! Me and a few friends rounded up 19 people in LA to get the antibody test, just out of curiosity. We all got the test from USC for about $95. This isn’t exactly scientific but here were are results:

Good news:

I have about twenty friends in LA and antibody testing is pretty easy to get (for $95)

We tallied 19 friends have been tested.
18 positive, majority with no symptoms.
1 negative.

I am positive this has been around for a long time.

We don’t know the long-term effects yet. The NYMag article was a clickbait scare-piece.

1. “Scientists report a case of a body infecting a live patient.” !!!
No ***** Pathogens don’t die as soon as the hosts do. There’s a reason dead bodies smell bad. There’s a reason people performing autopsies use PPE.

2. “The Coronavirus may have severe impacts on liver and heart function in some patients” !!!
No ***** So do a lot of other illnesses. Being sick can put strain on your heart. Respiratory illnesses in particular have a potential to cause cardiac issues (most are temporary). A lot of illnesses also cause temporary changes to liver function. These are well-known issues. We don’t have the follow-up data to show whether or not these changes are temporary or long-lasting following infection with Covid-19.

3. “The Coronavirus may attack T-cells, similar to HIV” !!!
This one pissed me off the most. The author used the phrase “more-harrowing evidence for the long-term consequences of contracting the virus.” This fear monger can go sit on a pole. He didn’t read the article and doesn’t understand key differences between HIV and Covid-19 or their effects on T-cells. There’s no evidence that Covid-19 has long-term effects on the immune system. None. It is not HIV.

4. “Trump’s touted treatment could increase risk for heart arrhythmias” !!!
Why was this even included in an article about the “Long-term Effects of the Coronavirus?”

The author, just like a lot of others, conflated chloroquine and hydroxychloroquine. They are two different drugs. The study also focused on severely ill patients taking different dosages of chloroquine and, as could be expected, the higher dose of chloroquine had more severe effects. Hydroxychloroquine has lower risk profile and is likely ineffective in helping people who are severely ill. It has the potential as a prophylactic and to help people who are moderately sick stay out of the hospital, but these uses are not the focus of recently published studies.

5. “COVID-19 could be causing kidney damage” !!!
A TON of acute illnesses cause acute kidney injuries and many people do just fine once recovered. Without long-term studies there’s no way to tell how long-term this damage is. But again, let’s run to the hills.

6. “It’s not all bad though” !!!
No mention of survival rate (very high) or people who do just fine. But contact tracing jobs are coming! Woo hoo!

The entire point of this article, as stated in the introduction, is that opening up the economy could be really scary.

Well, 4% ‘population immune’ is a disappointing number if it HAS been around a long time.

Either the immunity wears off, or it doesn’t travel fast to infect. One or the other. If it’s been around since November, 4 months *before* lockdowns started and only 4% of the population has immunity...that’s disappointing.

I get why they are doing it, but I have to say for me It feels a little too early for us in GA. We shut down later than most and had our second record number of cases and deaths today. We’ll see soon I suppose.
