HAMAS' Al-Aqsa TV inadvertently aired a satellite Polish erotic channel for six minutes before viewers were returned to normal programming.

In the early 1990s, a Master Control operator at WXYZ-TV channel 7 in Detroit was “monitoring” the video from one of the edit suites where they were putting together a news story about strip clubs. Needless to say, the footage was uncensored.
The operator inadvertantly put the source on air for several minutes...
