They should have a safe word (like Palomino) to let the others know that it is for real and not just an act.
There is probably still some critic who would pan the scene for lacking in realism.

(Sheesh, this could be a great story line for a play - - actor acts like he is having a real real heart attack while his character is acting that he is having a heart attack. Nevermind - - my head is already spinning.)
