The parent should be the one to ask their child if they want to speak with the doctor in private and the adults should respect the answer. Form or no form.
I do like that the form includes a question about learning how to say no to sex. In this day and age not including questions about being exposed to porn or being sexually harassed, intimidated or threatened via social media or smartphone seems a big miss.
I don’t have a problem with it overall provided the parent consents to the discussion and it is agreed that any revelation that could mean the child is in serious danger of harm will not be withheld from the parent. Parents also have veto power.
One thing I think should be included on such questionnaires is questions about dangerous driving habits.
Are you a licensed driver? “Have you ever driven when you were impaired in anyway? Have you ever been a passenger when the driver has been impaired in anyway? Do you or your friends ever drive at speeds more than xx MPH over the posted limit? Have you or your friends ever knowingly ignored stop signs or traffic signals? Do you keep your car up to date on its maintenance? Do you ever drive when you are distracted?” Teens and cars don’t always make a good mix. The odds of teen being injured or killed in an auto accident are a bit higher than the odds of them getting shot.
I know people get upset about the gun question. To me the doctor should simply ask the parent “Would you like to discuss firearm safety?” A yes answer does not necessarily mean there is a gun in the home.
Oh and why no questions about the perceived or actual crime rate where the family lives? Asking about local gang activity or whether they have ever been asked to join one would make sense too. I suppose that question and asking whether they or family members or friends have been a crime victim be considered (I don’t know) racist? The question should also apply to schools.
Another serious lack is no direct questions about being bullied and how that made them feel. We know that has a serious impact on a child.

When our kiddo was that age, the new doctor pull that same bs and with guns. Thankfully, I was in the room. Even better was kiddo came right back at the doctor and wasn’t playing her game. Daddy, being LE, obviously had guns. A phone call to the HMO five minutes later got us a new doctor.
The medical community wants everyone deemed mentally Ill and medicated.Be very careful what your children say or post online and who they confide in. Trust nobody.
Since the fake pandemic, and since my PCP pressured me to take the COVID placebo (vaccine), I now have ZERO trust in the medical profession. I remain unvaccinated, BTW.

i’ve never been asked about guns in the house, one ER tech asked me a list of q’s, one being “ do you feel safe”
She did not even finish asking me the q, she just rolled her eyes and continued.
They frequently asked me if we had guns in the house.
They never once asked me if we had a backyard pool.
Guns imo keep you safer but even if they didn’t a backyard pool is a far worse hazard.
It is not about safety obviously.
I have more of a backbone now. I would say something.

Yeah—this is big time mischief—certain answers could get the cops or social services called on the parents.
A kid who had just had a fight with their parents (or even just wanted to play a trick on them) could cause havoc.
