As Voyager should know in prior wars friendly fire casualties were much greater in number. I remember seeing something where in Germany WWII our artillery shelled a very large group of GI's killing hundreds.

B-1B Cockpit
The B-1B is operated by a crew of four: pilot, co-pilot, defensive system operator, and offensive system operator. The pilot and copilot are seated side-by-side in the front, behind large, sharply-raked windscreens which are designed to meet severe birdstrike specifications. The pilot and copilot both have fighter-style stick and throttle controls. The offensive and defensive system operators are seated side-by-side to the rear. Unlike in the B-1A, the rear crew members are each provided with a small side window. The crew enters the cockpit via a ventral hatch and extendable stairway that are located immediately behind the nose gear bay. Each of the four crew members sits on a Weber ACES II ejection seat. These seats replace the escape capsule used on the first three B-1As. Each seat has a survival kit, including a life raft and a radio beacon.

Unless Diego Garcia has changed a lot since 1978 when I was stationed there, its a 26 mile by 1 mile strip with a good airstrip, coral sand, tedium, and no women.

Give it to me. I will equip it with strip clubs, all-night casinos, a racetrack, powerboat and jetski bays, bars everywhere, internet cafes, and for income -- for export -- a factory that produced Saturday Night Special junk guns, illegal narcotics, switchblade and ballistic knives, and toilets that flushed with 2.6 gallons each flush!

That'd liven things up a little.

Not unreasonable that this was due to a refueling problem. Many heavy aircraft take-off with a limited amount of fuel so as to maximize ordinance. They refuel soon after take-off and possibly several times enroute during a mission. It is easier, and safer, with a full bomb load, to take-off with a limited amount of fuel and top-off as soon as reaching cruising altitude.

No need to get hysterical. Accidents happen even in peacetime. They are unavoidable. We have thousands of people killed each year just driving automobiles. I could probably walk out my front door and get killed by a falling icicle tomorrow morning. It is impossible to conduct a full scale war and obtain zero casualties.

I think that all considered, our casualties so far in this war have been astonishingly low.
