Patients treated with hydroxychloroquine have no better chance of survival than those who don't receive the drug hailed by Trump as a 'game-changer', results of a New York state Health Department trial suggest.
'I think from the review that I heard basically it was not seen as a positive, not seen as a negative,' said Governor Andrew Cuomo during CNN's coronavirus town hall.
Ultimately, the study, conducted by SUNY at Albany, is intended to involved some 4,000 coronavirus patients, but the preliminary results are from a sample of 600 patients.
Survival rates were no better among the group treated with the experimental drug than among those who got the standard supportive care, including oxygen, IV fluids and, if necessary, mechanical ventilation.
On Friday, shortly after preliminary results were reported, the Food and Drug Administration (FDA) formally warned against using the drug outside of hospitals due to the risk that it could cause heart arrhythmias.
It's a major blow to President Trump, who has sung the praises of the drug, as well as to the global search for an effective treatment for coronavirus.
More than 50,000 Americans have died of coronavirus, and 890,000 have been infected.
Officials and doctors alike have hoped that hydroxychloroquine, a drug developed nearly half a century ago to treat malaria, might improve coronavirus patients' odds by combating severe inflammation resulting from the viral infection.
In lab tests, the drug showed promise, appearing to quell the 'cytokine storm' of immune signalling cells that causes inflammation to run wild in the body, overwhelming the lungs.
Hydroxychloroquine interacts with the human immune response, making it useful in treating autoimmune diseases like lupus.
It was even added the official lists of treatments to try for doctors treating COVID-19 patients in China and South Korea,

Zinc levels were included in the “study”?
So basically it can go either way, some are helped, some see no improvement..I think it also depends on WHEN the drug is given, if you are already on a ventilator odds are nothing will help at that point

When are we going to see a study of HCQ given early not late?
I know there are some prophylactic studies going on among health care workers but I want to see studies along Zelenko’s protocol. Early treatment for over 60, high risk, or difficulty breathing. Include zinc.
I don’t trust Zelenko, I particularly don’t trust is claims about Brazil, that 3 hospitals following his protocol have reduced deaths by 95%. He appears to be the only source.
But let’s test his claims!

600 isn’t a large enough sample IMO...Not Even close.
They didn’t give zinc with the HCQ, which is the key part of the protocol that Dr. Zev Zelenko used. Looks like they were determined that the study would fail.

At some point we’re going to have accept that controlled studies with large groups of patients are the best way to confirm an intervention’s efficacy. Even better if they’re blinded and randomized in their allocation of study subjects. Of course, the timing of administration of HCQ might be critical, and it’s unclear if that was evaluated in this study. The Zinc issue? Who knows?

As with the VA study, it’s not made clear whether this is an apples-to-apples comparison of patients. How sick were the ones getting the drugs compared to the ones that were not, for example? It’s the first thing that should be asked, and in the VA case, it turned out to be an apples-to-cow pies comparison, i.e., a waste of time.
That would be because the survival rate is 99.9%.
People who got the drug probably would have survived anyway.
They also said the antibody tests aren’t reliable and should be ignored. Is this ground zero of the NY swamp?
The devil is in the details: In New York, no patient can be given hydroxychloroquine as an outpatients: this is per Cuomo’s executive order.
Thus, patients have likely been sick for at least a week, and in order to be hospitalized, they usually must have fever, respiratory compromise, or blood markers associated with severe disease.
Thus, in any New York study, we are looking at a sick population.
NoW, we know that when treating influenza with oseltamivir or baloxavir, TREATMENT MUST BE INITIATED WITHIN 72 HOURS OF ONSET OF SYMPTOMS.
Gilead has pointed out that their investigational drug, remdesivir, must also be given early in the Covid19 illness for a beneficial effect.
Uncontrolled observations of treating physicians is that hydroxychloroquine results is rapid resolution WHEN PATIENTS ARE TREATED EARLY IN THE DISEASE.
Thus, this result, in patients with advanced disease, is not surprising.
