I remember because I had sent my UIPA request for a certain document about the same time. I finally got answers from them which turned out to be bold- faced lies. Danae, unknowingly, provided proof of their lies to me a few months ago.
What happened to your requests? All of a sudden you were gone from, what was that web site that was posting your ordeal?
It is an official Certificate of live birth. I have no idea what a verification of live birth is or how to apply for one. That COLB was ordered by me and my island of birth was on the form which was complerly filled out by recollection of it I did not keep a copy of hat form. It was all done online, not through snail mail.
You want me to get ANOTHER Colb? I can’t answer why the island isn’t on there. I didn’t produce the colb. HDOH did. And everyone knows which island Honolulu is on for heavens sake. Whats it matter if its missing? Hawaii HDOH is FULL of inconsistancies!!
I am reluctant to do anything you ask at this point. I don’t think it will do any good. Certainly not enough to warrant ANOTHER 10 bucks+hassle. You are really gonna have to sell me on doing that at this point.

The receipt has 'Cert.' checked rather than 'Verif.' That's one thing that tends to lend some credibility to dana's documents. The layout of that receipt is pretty elaborate.
Also, I'm curious why you're taking exception to the 'Date Filed' language. I'm pretty sure this was part of a national standardization effort for vital records offices. Dr. Alvin T. Onaka participated in committees that recommended new standards in the late 1990s and early 2000s. It's partly how we get COLBS now instead of LFBCs.

It matters if Oahu was present or not. The HDOH will only verify what is presented to them on an application to verify. If you didn’t ask for Oahu, Oahu wouldn’t appear.
And if that’s the case, you have a ‘Verification’ not a Certificate. In 2001, The Legislature passed this bill:
“Vital Records; Certificate Information
Description:
Allows for verification and certification of the existence of a birth, death, marriage, or divorce certificate and the essential facts of the vital event that are contained in the certificate in lieu of the issuance of a certified copy of the certificate.” http://www.capitol.hawaii.gov/session2001/bills/SB1123_cd1_.htm
The HDOH Website says this:
“The verification process will not, however, disclose information about the vital event contained within the certificate that is unknown to and not provided by the applicant in the request.” http://hawaii.gov/health/vital-records/vital-records/vital_records.html
And if you check your receipt, you will see that you can order either a certified copy or a verification. Without the word ‘Oahu’ on your ‘Certification,’ it appears your COLB may not even be a copy of a certificate—which is something that is ON RECORD with the state.
You don’t have to do anything more, Danae. Not for me anyway. I would not expect you to.
I haven’t see a receipt for the ‘Certification of Live Birth’ that’s why I am asking.

Were you simply cynical, you could’ve civilly asked the

I was closely watching MissTickly’s (aka TerriK) crusade against HDOH last year because I also had made a UIPA request and they were not responding to me either. All of a sudden POOF she was gone. I always wondered if she ever got any information from them. Or if she was threatened by BO’s goons.
I don’t want her to take it personally if she is on the level.
Take it for what it’s worth. I am suspicious of everyone on the internet. To be anything other would be stupid.
