If you don’t know the answers, can you try and find out from Onaka or Fukino if your ‘Certification of Live Birth’ with the missing field ‘Oahu,’ and the ‘Date Filed By’ language is really just a ‘Verification’ of your birth facts from a file maintained by the Health Department rather than an actual abstracted short form copy of your birth certificate which is ON RECORD with the Office of Health Status Monitoring?

The receipt has 'Cert.' checked rather than 'Verif.' That's one thing that tends to lend some credibility to dana's documents. The layout of that receipt is pretty elaborate.

Also, I'm curious why you're taking exception to the 'Date Filed' language. I'm pretty sure this was part of a national standardization effort for vital records offices. Dr. Alvin T. Onaka participated in committees that recommended new standards in the late 1990s and early 2000s. It's partly how we get COLBS now instead of LFBCs.
