I had a similar event January 15th. Immediately stopped any blood thinning substances (ibuprofen, nattokinase, serrapeptase, turmeric). A CBC/electrolytes + ferretin test showed me stage 3 iron deficiency anemic. March 14th a colonoscopy and transoral endoscopy was performed. An ulcer in the duodenum was healing (likely the source of the bleed) after 60 days with no thinners. 7 per-cancerous polyps were removed. I'm still anemic as of April 15th even with iron supplements and no detectable bleeding.

In additional to immediate cessation of blood thinners, I was put on a proton pump inhibitor to reduce stomach acid in hopes that the ulcer would heal. It did.

The transoral endoscopy included checks for H. pylori infection of the stomach (none) and celiac sprue in the duodenum (none).

Having just been through the gauntlet, I don't envy what your husband has ahead, but hopefully it can be narrowed down to a treatable, recoverable state. Mine went further down the rabbit hole leaving me with a diagnosis of adenocarcinoma of the ampulla of Vater. The road ahead doesn't look great.
