My Son Samuel, is an attorney and for friends, has written the following request for exemption, to use in work situations. I can’t guarantee it will work, but he puts the issues well and succinct. I asked for permission to post this and he granted it…
To whom it may concern: I am writing to notify you of my religious and philosophical objections to the COVID-19 vaccine, and to request a reasonable accommodation and exemption on the basis of those objections.
As a Christian, I believe that all life is sacred and bears the image of God. Consequently, I believe that the act of abortion is deeply immoral.
I cannot in good conscience receive any vaccine which uses, benefits from, is derived from, or is in any way connected to fetal cell lines. For me, to receive the vaccine would be to endorse and turn a blind eye to what I believe to be a grave sin.
Additionally, please understand that the principles of bodily autonomy, bodily integrity, and informed consent are central to my worldview. I am deeply opposed to any mandate that would result in the non-consensual penetration of my body as a condition of employment–especially in the absence of any long term, peer reviewed studies relating to potential side effects of the COVID-19 vaccine.
Based on my sincerely held religious and philosophical beliefs, I cannot in good conscience receive any of the currently available COVID-19 vaccines.
Furthermore, I understand that the right to live in accordance with my convictions free of discrimination or coercion in this context is protected by a large, well-established body of law, including but not limited to the First Amendment to the United States Constitution, the Civil Rights Act of 1964, the Federal Food, Drug, and Cosmetic Act (and other federal law), as well as Iowa constitutional and statutory law.
Based on the foregoing, I am hereby requesting a reasonable accommodation and full exemption from any Covid-19 vaccine mandate.

Well done. My employer accepted a statement somewhat shorter. This sample adds/covers the essentials of autonomy as covered by current law.

Just an FYI for all: I used a religious exemption for my daughter all during school and for myself in college. Be aware they will lie to you and say “We can’t accept this.” You have to stand firm and demand they accept it. I would reply, “Yes, you can.” We’d go back and forth a bit but they always eventually backed down. They look at you like you’re a scraping from the bottom of a shoe but stand firm. If all else fails, demand any refusal be in writing. No one wants to put their name on a piece of paper these days.
My friend works for a health organization that specifically stated that the exemption request could not use abortion in the request. It would be immediately denied. They had all sorts of limitations. Of course, as of now, there have been zero approved.
A suggestion to consider remodeling any philosophical wording unless your states allows philosophical exemptions.
I simply don’t understand that. Some companies have religious exemptions but it is so narrowly tailored that it rarely is approved? I don’t understand how that is not a potential civil rights violation.
