I've watched plenty of courtroom videos. If the stenographer has trouble making out what the witness is saying, she signals the judge, who admonishes the witness to slow down and speak clearly.

Also, there are always a prosecution and a defense attorney present, who will demand a clarification if something important isn't clearly stated for the record.
