The collisions were avoidable between USS FITZGERALD (DDG 62) and Motor Vessel ACX CRYSTAL, and between USS JOHN S MCCAIN (DDG 56) and Motor Vessel AALNIC MC.
Three U.S. Navy investigations concerning each of these incidents are complete.

Uh, that would be a good start. Prayers up,for the lost folks in both cases. And the crews that saved both ships.

The OOD should practically be court-martialed, but the CO should've never allowed them to drive the ship on their own. I speculated quite a bit based on the early reports of this accident; after reading the final report, I can't believe the sheer incompetence that permeated throughout this command.
