Update: After only a month of criticism—including from Nomad author Ayaan Hirsi Ali, below—the American Academy of Pediatrics has reversed a policy that advised American doctors to give a ceremonial pinprick to girls of immigrant families so that they would avoid seeking a full circumcision.

The American Academy of Pediatrics recently put forward a proposal on female genital mutilation. They would like that American doctors be given permission to perform a ceremonial pinprick or “nick” on girls born into communities that practice female genital mutilation.

Female circumcision is a custom in many African and Asian countries whereby the genitals of a girl child are cut. There are roughly four procedures. First there is the ritual pinprick. This is what Pediatrics refers to as the “nick” option. To give you an idea of what that means, visualize a preteen girl held down by adults. Her clitoris is tweaked so that the circumcizer can hold it between her forefinger and her thumb. Then she takes a needle and pierces it using enough force for it to go into the peak of the clitoris. As soon as it bleeds, the parents and others attending the ceremony cheer, the girl is comforted and the celebrations follow.

It’s funny that NOW and the other women’s groups scream bloody murder when someone suggests that abortion is murder, yet seem to be largely silent about barbaric acts such as this.

Commonly referred to as female genital cutting or female circumcision, a clitoridectomy is usually performed for cultural or religious reasons, which may vary from culture to culture. In some areas, the surgery is thought to maintain cleanliness, while others believe removing the clitoris will prevent women from engaging in premarital sex. Although practiced throughout the world, the procedure is most common in parts of Asia and Africa.
The procedure is often accompanied by infibulation, or the stitching together of the vulva. This is usually done following the removal of the clitoris, when the woman’s labia major is sewn together, leaving an opening small enough for only urine and menstrual blood to pass through. Prior to marriage, the opening must be enlarged to allow for penetration during intercourse, a process that can take three or more months.

In addition to sexual dysfunction, a clitoridectomy has many long-term effects including incontinence, sterility, increased susceptibility for the AIDS virus, and even death. Medical complications are common since the person who performs the procedure is generally not medically trained, but rather a parent or elderly member of the community. The surgery is usually performed without anesthesia, with a razor, glass shard, or other unsterilized instrument. As a result, tetanus, hemorrhages, and massive scarring can also occur. There are also long-term psychological effects, including depression, anxiety, and reduced self-esteem.

Most women do not choose to undergo a clitoridectomy, as the surgery is typically performed on girls between the ages of four and eight. These girls are usually tied or held down during the procedure. Estimates suggest that approximately 130 million women and girls across the globe have been forced to have the surgery. Now considered a form of violence against women, the clitoridectomy has been banned in many areas of the world. Despite this, however, many cultures continue to practice the procedure.

They do it this way because there are more interested in preventing crippling genital mutilations than scoring rhetorical points.

Personally, I’d be pleased if our own culture had done the same instead of chopping chopping off part of the end of my penis (and the majority of mens’ as well) at birth, for what amount in many cases to “cultural” or “religious” reasons:

“Virtually all current policy statements from specialty societies and medical organizations do not recommend routine neonatal circumcision, and support the provision of accurate and unbiased information to parents to inform their choice.”

‘The little girl, entirely nude, is immobilised in the sitting position on a low stool by at least three women. One of them with her arms tightly around the little girl’s chest; two others hold the child’s thighs apart by force, in order to open wide the vulva. The child’s arms are tied behind her back, or immobilized by two other women guests. The traditional operator says a short prayer: “Allah is great and Mahomet is His Prophet. May Allah keep away all evils.” Then she spreads on the floor some offerings to Allah: split maize, or, in urban areas, eggs.

Then the old woman takes her razor and excises the clitoris. The infibulation follows: the operator cuts with her raxor from top to bottom of the small lip and then scrapes the flesh from the inside of the large lip. This nymphectomy and scraping are repeated on the other side of the vulva. The little girl howls and writhes in pain, although strongly held down. The operator wipes the blood from the wound and the mother, as well as the guests, “verify” her work, sometimes putting their fingers in. The amount of scraping of the large lips depends upon the “technical” ability of the operator. The opening left for urine and menstrual blood is miniscule.

Then the operator applies a paste and ensures the adhesion of the large lips by means of an acacia thorn, which pierces one lip and passes through into the other. She sticks in three or four in this manner down the vulva. These thorns are then held in place either by means of sewing thread, or with horse-hair. Paste is again put on the wound.

But all this is not sufficient to ensure the coalescence of the large lips; so the little girl is then tied up from her pelvis to her feet: strips of material rolled up into a rope immobilize her legs entirely. Exhausted, the little girl is then dressed and put on a bed. The operation lasts from fifteen to twenty minutes according to the ability of the old woman and the resistance put up by the child.’

“The practice seems to have originated in Egypt, during the Pharaohs’ time and could have been spread across Africa with the Arab slave traders.

There are 4 types of mutilation, related to how severe the operation is. Types I and II are less severe. Type I (Sunna circumcision) means clitoridotomy (removal or splitting of the clitoris hood).

Type II means clitoris removal (clitoridectomy) with the total or partial extirpation of the labia minora (inner lips). The sewing together of the leftover labia minora epidermis, which contains sweat glands, favors a buildup of sweat and urine in the closed off space beneath this closure that can lead to local or urinary infection, septicemia, hemorrhaging and cyst formation.

Types I and II are the most common, encountered in all Western Africa (Mauritania, Senegal, Gambia, Mali, Guinea, Sierra Leone, Liberia, Ghana, Ivory Coast, Burkina Faso, Niger, Chad, Togo, Benin, Nigeria), plus Central Africa (Camerun, northern Zaire, Central African Republic, southern Sudan), Egypt, Ethiopia, Kenya, northeastern Tanzania, eastern Uganda and southern Arabian peninsula (Yemen, Oman) in a minority representing 20 % of the population.

The most severe types are III and IV, which represent 15 % of the total. Type III (infibulation, Pharaonic circumcision) means the partial or total removal of the female external genitalia and suture of the vaginal opening. Type IV means the puncture, perforation or incision of the clitoris and/or labia. These types are practiced in northern Sudan, Eritrea, Somalia, eastern Ethiopia and neighboring zones.

The infibulation involves massive tissue removal of the external genitalia, including all of the labia minora and the inside of the labia majora, leaving a raw open wound. The labia majora are then held together using thorns or stitching and the girl’s legs are tied together for two - six weeks, to prevent her from moving and to allow the healing of the two sides of the vulva. Nothing is left of the normal anatomy of the genitalia, except for a wall of flesh from the pubis down to the anus, with the exception of a pencil-size opening at the inferior portion of the vulva to allow urine and menstrual blood to pass through.

When girls are married, in many cases with much older men, to have sexual intercourse, the husband must use a knife on the wedding night to widen the aperture� During childbirth, the enlargement is too small to allow vaginal delivery, and so the infibulation must be opened completely and restored after delivery. Once again, the legs are tied together to allow the wound to heal, and the procedure is repeated for each subsequent act of intercourse or childbirth.

When childbirth takes place in a hospital, the surgeons may preserve the infibulation by enlarging the vagina with deep episiotomies. Afterwards, the patient may insist that her vagina be closed again so that her husband does not reject her.”
