In an effort to curb the rapidly increasing health threat of diabetes in New York City, the Bloomberg administration earlier this year mandated that medical laboratories report the results of special diabetes-monitoring blood tests, along with the name and contact information of patients, to the City Department of Health. While efforts to reduce the devastating health consequences of diabetes are laudable, the violation of patient confidentiality and the precedent set by this new regulation should be the source of grave concern to all New Yorkers.

Diabetes is now among the leading causes of death in the city - and its incidence is rapidly increasing. Genetics (family history) plays a major role - African Americans, Asians, and Hispanics are much more susceptible to diabetes than whites, for instance. Obesity is a major risk factor for the disease. If not managed prudently, diabetes causes kidney failure, heart attacks, strokes, and other life-threatening or debilitating illnesses.

Given that diabetes prevention (through weight control) and management (through diet planning, exercise, monitoring, and medications) are matters of personal commitment and responsibility, the disease cannot be "solved" by government intervention that goes beyond educational programs. But that is exactly what this new city mandate does: not only is personal medical data now being reported to city officials, but the Department of Health is now doing what they call "interventions": diabetics will receive letters and phone calls from city officials offering guidance on dealing with the disease. If you wish to keep your medical data confidential, you cannot. If you want to avoid "interventions," you must have sophistication and knowledge enough to fill out forms requesting that you not be contacted.

This new diabetes regulation is, in short, a harbinger of more intrusive legislation to come - all in the name of "public health."

Preposterous...just simply astounding. What will it cost the taxpayers of NY to run this harassment program? It sounds like the government there needs some "interventions" from the citizens.... Send them all home at the next election.

I think I might go get a diabetes test performed in the city. Won't want to miss out on the class action settlement on this one.

It will be a federal class action suit too, and will probably have jurisdictional change of venue.

This is a HIPAA violation that could bring the doctors/clinics/hospitals who don't have patients sign confidentiality forms brought in as co-defendants until such time as they can prove they were obligated under law to perform the violations.

This will end up a bloody fiasco for the city.

Diabetes is awful. Especially getting your feet amputated. People just need to take care of themselves and take things in moderation.

Mastoparan, a tetradecapeptide from wasp venom, stimulates insulin secretion from the islet ß-cells, presumably via activation of trimeric G proteins. Herein, we used Clostridial toxins, which selectively modify and inactivate the Rho subfamily of G proteins, to examine whether mastoparan-induced insulin secretion also involves activation of these signaling proteins. Mastoparan, but not mastoparan 17 (an inactive analog of mastoparan), significantly stimulated insulin secretion from ßTC3 and INS-1 cells. Preincubation of ßTC3 cells with either Clostridium difficille toxin B, which inactivates Rho, Cdc42, and Rac, or Clostridium sordellii toxin, which inactivates Ras, Rap, and Rac, markedly attenuated the mastoparan-induced insulin secretion, implicating Rac in this phenomenon. Mastoparan-stimulated insulin secretion was resistant to GGTI-2147, a specific inhibitor of geranylgeranylation of Rho G proteins (e.g. Rac), suggesting that mastoparan induces direct activation of Rac via GTP/GDP exchange. This was confirmed by a pull-down assay that quantifies the binding of activated (i.e. GTP-bound) Rac to p21-activated kinase. However, glucose-induced insulin secretion from these cells was abolished by toxin B or GGTI-2147, suggesting that the geranylgeranylation step is critical for glucose-stimulated secretion. Mastoparan significantly increased the translocation of cytosolic Rac and Cdc42 to the membrane fraction. Confocal light microscopy revealed a substantial degree of colocalization of Rac (and, to a lesser degree, Cdc42) with insulin in ß-cells exposed to mastoparan. Further, stable expression of a dominant negative (N17Rac) form of Rac into INS-1 cells resulted in a significant reduction in mastoparan-stimulated insulin secretion from these cells. Taken together, our findings implicate Rho G proteins, specifically Rac, in mastoparan-induced insulin release.

The same markers that are used to show infection may cause heart attacks are also abnormal in diabetes that starts later in life. When a germ gets into your body, your immunity sets off a large number of chemical reactions aimed at killing that germ. Almost everyone agrees that diseases of inflammation are often started by infection, but as the disease progresses, doctors often cannot find an infectious agent. Now we find that diabetes that starts in later life has an overactive immune system just like rheumatoid arthritis, late-onset asthma, Crohn's disease and heart attacks.

Mears, I see this happening time and time again.  These idiots promise us the moon to get our votes.  Once in office all they hand us is the out house!

The power goes to their heads and they can't get over their love of "ruling the little people."  Bloomberg is just another power hungry grubber lording it over the people of NYC and he is LOVING it! I think he even has Governor Pataki in his pocket.
