Thrown within 100 feet of Bush on stage. Pin was pulled. Georgian securiy got it. Thats all from NBC news.

I can't find any hand grenade that is effective out to 100 feet, so it never got close enough to even wound him.
By the way, I was impressed with the press conference yesterday, the way the Georgian president spoke in English. He was unabashedly supportive of W and the US.
