Fish oil supplement halves serious cardiovascular events in patients on dialysis, clinical trial finds
Medical Xpress / Monash Univ / Am Society of Nephrology Kidney Week 2025 / New Eng Jrnl of Med ^ | Dec. 8, 2025 | Charmaine E. Lok et al

A daily fish oil supplement has been shown to significantly reduce serious cardiovascular events in people receiving dialysis for kidney failure. The findings come from a major international clinical trial.

The PISCES trial involved 1,228 participants across 26 dialysis sites in Australia and Canada.

Participants who received four grams per day of fish oil, containing the natural active ingredients EPA and DHA, experienced a 43% lower rate of serious cardiovascular events compared with the placebo group. These events included heart attack, stroke, cardiac death and vascular related amputations.

Adjunct Professor Kevan Polkinghorne, "Patients on dialysis have extremely high cardiovascular risk, and very few therapies have been shown to reduce that risk," Professor Polkinghorne said. "In a field where many trials have been negative, this is a significant finding.

"Dialysis patients typically have much lower levels of EPA and DHA than the general population. This may help explain the magnitude of benefit observed in this group."

Professor Polkinghorne noted that the findings are specific to people receiving hemodialysis for kidney failure and should not be applied to healthy individuals or other patient groups.

Central trial coordination was provided by the Australasian Kidney Trials Network (AKTN). About 200 Australian participants contributed to the study, including 44 treated at Monash Health.

A really good fish oil supplement, or a diet higher in fish-based Omega-3s, greatly helped dialysis patients to not get cardio issues.

Interesting. I keep hearing good things about fish oil,
so last night, I opened a can of Alaska salmon and consumed its contents in their entirety.

I buy Icelandic cod liver oil on AMZ. Iceland has the cleanest water in the world, so cod are really healthy. Atlantic Ocean fish are to be avoided, filthy water. Sad.

All my ocean fish are caught in NC.
My wife and I eat lots of smoked salmon from the NW.
She bought me a can as a reminder of my bachelor days
when my place smelled like fried fish or Zatarain’s.

I have been on 3 grams (need to up it to 4) of EPA supplements for five years. This fish oil supplement specifically removes DHA. Essentially it is over the counter RX Vescepa which is FDA approved for reduction of cardiac events. Proven to reduce cardiac events via reduction of arterial plaque accumulation and with sustained use reduces existing plaque. No idea if it’s helping me other than I’m still alive and no heart conditions other than high BP.

“This fish oil supplement specifically removes DHA”

I should have further explained that DHA and EPA omegas are found in OTC fish oil supplements which on their own in combination do reduce triglycerides. But I don’t know that triglycerides are the boogy man that we all heard about 10 - 20 years ago. In combination these two omegas don’t reduce arterial plaque, no synergy there. Only pure EPA omega has shown plaque reduction.

Update to update. I buy Carson EPA Elite Gems when Swenson runs their 30% off sale and more recently GNC brand triple strength 1000 EPA. GNC has seemingly a permanent buy one get one 50% off. Occasionally a buy two get one bottle free. Being a reoccurring customer I get emailed a $5 off coupon off every other month or so and another 15% discount for picking it up at a brick and mortar vs having it shipped.

GNC works out a bit cheaper than Swenson even with the 30% discount Swenson runs fairly often.

At 4 grams a day it isn’t inexpensive, about $50 a month. If your Dr will put you on RX Vescepa or the generics it should cost less, maybe nothing with Ins.

Make sure a true and not rancid fish oil supplement otherwise the benefits are negated by the inflammation rancid or lesser quality ones can cause.

I keep hearing/reading better to make your one fish/bone broth from oily skins and larger bones or just eat skins of cooked oily fish and its usually better than supplements and cheaper.

I'll usually pick up the Carlson Elite Omega-3 gems, as well.  I've tried Wiley's Finest, too, but they're generally more expensive so I usually stick with Carlson.  Neither gives me any kind of fish burps.

I've read good things about Nordic Naturals, but I don't think I've ever tried those.

The paper’s abstract fails to identify whether the fish oil was commercially available OTC supplement or laboratory derived fish oil.

The difference is of paramount importance for study validity, the reason for which I’m not delving into here.

The study I’d like to see - I won’t hold my breath - is for patients consuming fatty fish vs a control group consuming commercially available fish oil supplements to confirm the efficacy of the latter.

Now THAT would be a study with Earth-shaking results.

I’ve eaten enough tuna fish over the years to feed a small country. I’m sitting here 3 months post op from quadruple bypass surgery and recovering from a defibrillator implant I got last Thursday. Fish oil did not work for me.
