There are several strategies for the early detection of prostate cancer. The first step is often a blood test for prostate-specific antigen (PSA). If PSA levels exceed a certain threshold, the next step typically involves taking a tissue sample for analysis. Another option is to use magnetic resonance imaging (MRI) to search for signs of a tumor before deciding whether a biopsy is necessary.
Researchers conducted a study to determine whether this MRI-first approach is safe over the long term. Their findings show that this strategy poses no additional risk to patients for at least three years.
This approach, in which normal MRI findings are followed by regular urological checks, did in fact prove to be sufficiently reliable: The study found that 96% of patients with a normal MRI result would not go on to develop aggressive prostate cancer within three years. Aggressive prostate cancer was detected during further monitoring in just 4% of participants whose initial MRI findings had been negative.
The team included and monitored nearly 600 patients with suspected prostate cancer in their study. The subjects underwent multiparametric magnetic resonance imaging (mpMRI). This type of MRI detects multiple tissue-specific parameters, including the signal intensity of the prostate tissue, the blood flow or perfusion, and the diffusion of water molecules in the tissue.
"Tissue samples were taken only if the MRI showed suspicious findings in the prostate. Patients with normal MRI findings underwent regular urological check-ups for three years instead," Dr. Hamm says.
The authors say two other aspects are crucial. First, a high-quality MRI scan must be performed and analyzed by experienced experts; that means training more radiologists in the interpretation of prostate MRI scans and using standardized methods. Second, it is important to create a safety net for patients who do not undergo immediate biopsy.

The doctor told me that biopsy wouldn't be too bad, it would be like two fingers, not one. It wasn't like two fingers; it was more like a football. At least it was pointed on the ends so it would go in easier. I've had more of these than I can count. I think I'm getting better at passing a football than Tom Brady. Actually, the last two were done through the perineum and under light sedation. This was said to be safer at avoiding infection than with the football.
My guy has also put on weight since his radiation and anti-testosterone. He tries to eat well, but consumes more carbs than he should to lose weight. Enjoy your meat this holiday. Avoid the potatoes, white and sweet, and the desserts. Enjoy the cooked greens with butter, and the raw greens with low carb dressing. Several dressings have 1 or 2 carbs per 2 tablespoons. Others like Thousand Island are 4 grams of carbs or higher. He also used to pig out on barbaque sauce with 12 or 14 grams carbs per 2 tbs. On several occasions I have decided to get serious about weight loss. By limiting my diet to no more than 20 grams of carbs a day I have achieved the desired weight reduction. Of course, then I get a bit careless and the weight drifts back up, but the low carb diet does work when I apply it.
I started with the PSA test and then my urologist prescribed an MRI followed by a biopsy. I thought it was a rather standard regimen.

I had the MRI guided biopsy twice. It seems to be effective, as cancer was found (fortunately the mildest kind). I was still doing testosterone replacement at the time - doc told me to stop it and put me on active monitoring. Since then my PSA has gone back to normal range. The biopsies suck - but it’s nice to know what’s in there.

The last thing I want to see is another man who is happy and adamant about digitally probing my anus. I am done with doctors that I think dont know squat, experimenting with my health on their 9-5 job.
