Why lie about this? Complications happen in any endeavor, and in space, I’d imagine it’s quite common.
I’m assuming the feds are so used to lying and cover-ups, that it’s basically a reflex at this point.

I'm sure they practice emergency response all the time. But their "reflexive lying" is my #1 position about them. FedGov serves only one master -- itself. They lie to their bosses (us) all the time.

Training exercise, huh? Color me skeptical. Really tired of our government hiding everything from us. There would be no conspiracy theories if they would just be honest.We have a right to know everything except those things that would threaten national security or private information about individuals.
