As ground workers began to approach, the flight’s first officer opened a cockpit window and gave the workers a verbal warning, according to the report.
Emergency lights and a “crew alerting system” were also in operation.
Nevertheless, one worker walked in front of an engine, was “pulled off her feet and into the operating engine” and suffered fatal injuries.
The ground crew worker — later identified as Courtney Edwards, 34, according to Fox News — was a mother of three.
Edwards was given multiple warnings that the engines would remain on during the cool-down period in addition to the pilot’s verbal warning and the safety lights.
According to the NTSB review, the MGM ground crew held multiple safety meetings in advance of the plane’s landing.
During these meetings, it was emphasized that the engines would be running, and therefore unsafe, during the engine cool-down period, which was set to last until the plane connected to ground power.
As Edwards approached the plane to set up cones and perform her duties, multiple crew members attempted to wave her off.
The NTSB review noted that The American Eagle Ground Operations Manual clearly warns ground crew operators not to approach an aircraft while the engines are on and the rotating beacon (one of the warning lights ignored or unseen by Edwards) is flashing.

I have seen people ignore safety (and safety officials - which I am). It seems to be a worker’s perogative to do things like this and the usual justification is that if the safety regs are followed to the letter then the work is slower and or harder. This does not make sense, but that is the answer to Why? in most cases like this.
I’m respectfully laughing, is that ok? Too stupid to breathe comes to mind. That engine will have to be rebuilt and the plane out if service. So look at the bright side, no funeral service or casket costs. And the three kids can be raised by their father for the betterment of society.
Ms. Edwards probably was hefty enough.

Re: 51 - The LOR was what I heard after I PCS’d from that base.
Apparently he was flagged for previous safety violations so that may be a reason for an LOR. If so, not sure how he got another aircraft (that I know he did).

It was nice and messy up there in 90-95 degree weather. Just like an oven!
I’ve worked in machine shops long enough to know that ultimately, safety was MY responsibility.
OSHA is a government agency that wouldn’t be needed if everyone who worked in a hazardous environment would use common sense. In some cases OSHA isn’t needed, but that government overreach is another subject.

Yep. I got to the mill to go see the storeroom guys and the millwrights. Was not allowed in the plant. LOTS of law enforcement, fire fighters and paramedics. Really shook up the guys who had worked there for a long time.
Are you talking to me? Seems like a LOT of people these days can NOT connect behavior to longevity. I will defer to comment further.

He thought he did it “correctly” by disobeying the explicit instructions to install the bolts “upside down” (he believed).
