A study suggests that some patients diagnosed with behavioral-variant frontotemporal dementia (bvFTD)—an incurable condition that robs patients of the ability to control their behavior—may instead have a cerebrospinal fluid leak, which is often treatable.
Cerebrospinal fluid (CSF) circulates in and around the brain and spinal cord to help cushion them from injury. When this fluid leaks into the body, the brain can sag, causing dementia symptoms. Schievink said many patients with brain sagging—which can be detected through MRI—go undiagnosed, and he advises clinicians to take a second look at patients with telltale symptoms.
Clinicians can also ask about a history of severe headaches that improve when the patient lies down, significant sleepiness even after adequate nighttime sleep, and whether the patient has ever been diagnosed with a Chiari brain malformation. Brain sagging, Schievink said, is often mistaken for a Chiari malformation.
Even when brain sagging is detected, the source of a CSF leak can be difficult to locate. When the fluid leaks through a tear or cyst in the surrounding membrane, it is visible on CT myelogram imaging with the aid of contrast medium.
Schievink and his team recently discovered an additional cause of CSF leak: the CSF-venous fistula. To detect these leaks, technicians must use a specialized CT scan and observe the contrast medium in motion as it flows through the cerebrospinal fluid.
Investigators used this imaging technique on 21 patients with brain sagging and symptoms of bvFTD, and they discovered CSF-venous fistulas in nine of those patients. All nine patients had their fistulas surgically closed, and their brain sagging and accompanying symptoms were completely reversed.
The remaining 12 study participants, whose leaks could not be identified, were treated with nontargeted therapies designed to relieve brain sagging. However, only three of these patients experienced relief from their symptoms.

Pretty good news- alzheimers and such are terrible things to endure- and hard on caretakers- extremely hard- problem though will be getting a doctor to agree to look for such things- they tend to not like people bringing ‘internet info’ to them- even if that info cites respected medical institutes like Cedars Sinai- at least that’s been my experience-
So my 20 minute mid afternoon power nap is a bad thing?
I started doing them a while back. I read an article that a 20 minute nap with a timer allows you to be fully refreshed without going into a deeper sleep state which will make you wake up groggy and usually is one of those two hour jobs.
So I started putting the timer on my phone on and laying on the floor with my feet up on the sofa and take a 20 minute nap, it’s just enough where the edge is off and I feel great.
I’ll bet his Doctors are looking into this, hard core!

Three years ago I went to the doctor for some odd symptoms which appeared to be a brain problem. They sent me for a cranial scan. The technician who performed the scan called me into an office at the lab and told me to immediately go to the hospital across the street.
They checked me in and kept me for diagnosis. After three days, two doctors could not agree about what was wrong. My wife prayed that they would send me home. Knowing that I had a major problem, they did send me home.
My wife got me an appointment with my GP the next day. He immediately suspected a CSF leak and called for a consult with Barrow’s clinic in town. (Phoenix) The doctor from Barrows gave him some simple tests to give, then made an appointment the next day.
By the time I got to Barrows, I was slipping into a comatose state. Barrows sent me to St. Josephs next door to them and scheduled scans the next day.
The scans showed that so much cranial fluid had leaked out that my brain was resting on bone. They located the spinal leak and sealed it so that my spinal fluid could replenish. My full recovery took about five months.
The take-aways for this story are that God arranged for a lot of things to line up so that my live was saved by the Barrows doctors. My GP said that the doctors at the first hospital were following standard protocols, which were inadequate. Protocols do not properly handle CSF leaks!
