CKD patients with renal failure (stages 2, 3, 4) were recruited as volunteers for a randomized double blind and placebo control pilot study. All patients suffered from diabetic nephropathy and hypertension. All patients were asked to keep their habits and medication as usual and were dispatched into two groups: experimental and placebo.
The GSE group was daily supplemented with six capsules of GSE, each capsule containing 350 mg of grape seed powder. Two bottles, 90 capsules each were provided to each patient for one month during the entire six months long study. The placebo group received starch.
GSE was processed from a grape cultivar (Carignan) of Vitis vinifera from northern Tunisia. Seeds were carefully separated from skin, washed, dried and grounded with an electric grounder until a fine powder was obtained. After mixing with 3 % colloïdal silica as excipient, the powder was conditioned into capsules weighting approximately 350 mg.
GFR was calculated by the Modification of the Diet Renal Disease (MDRD) method.
GSE increased 1/plasma creatinine ratio by 19.42 %, p = 0.044), and decreased proteinuria by -32.67 %, p = 0.041) but had no significant effect on plasma urea and uric acid. Moreover GSE slightly improved GFR by +18.70 % (36.74 ± 5.74 versus 43.62 ± 3.68, p = 0.604)
First of all GSE improves some kidney function parameters as it enhances GFR and clearly lowers proteinuria. This result is of utmost importance as reduction in proteinuria to the lowest achievable level is an important predictor of long term renal protection.
Importantly no patient drop out of the study nor adverse side effects were noted during the entire clinical trial period. Our data corroborate several previous works in the field demonstrating the heightening of oxidative and inflammatory biomarkers along with a decrease in antioxidant biomarkers in CKD patients.

The translation doesn’t always make complete sense, such as using “GFR” when it was really “eGFR,” and saying “Grape seed powder” in the title, while using “Grape seed extract” or “GSE” throughout the paper. Based on the explicit information given, it was absolutely only “grape seed powder” and “eGFR” used.
About the “grape seed powder,” the likely amount of polyphenols in that would be something less than 600 mg of grape seed extract, from what I have found elsewhere about the total content of grape seed powder, which includes fiber and protein for major components.
You can buy grape seed powder, but most would find it easier to find grape seed extract.. It could be helpful to take it as many as five times a day, like the powder was dosed, as the polyphenols are not rapidly taken up by the body, so you could consider subdividing up your capsule(s) accordingly. I believe fat may help it be absorbed, as well.
I have been taking grape seed extract each day for over 18 years. My current dosages are either 300 or 400 mg a day, depending on specific type of grape seed extract I buy. I have taken over 5,500 mg a day for five days at a time, twice, as an experiment, and this was tolerable when I incrementally took it through the day, but definitely not all at once, which I did one time only. I am not suggesting anyone else do such a thing, though! I only offer this as insight into its apparent safety for me, when I was younger. For anyone with a kidney or other health issues, please talk with your doctor when taking any new supplements, as some can cause issues with medicines (or even other supplements).

I’m sure it depends on the cause. In my case, with my glomeruli reduced to scar tissue, this would absolutely do nothing for me.
I wish I had known of this possible treatment some years ago when my Dad was still alive. Renal Failure was one of the things that did him in. However, his doctors were typical American Doctors. They have likely never been taught to use this method of treatment. They would have politely told me to leave to them the actual professionals.

Except that niacin has a widely-reported side effect, the “niacin flush”, that some find bothersome and objectionable. This report made specific note of the absence of an adverse side effect.
For whatever reason, I’ve always been susceptible to the annoying effects of Niacin flush. Two months ago, a Freeper recommended Zero Flush Niacin from “Finest Nature” and I now do daily 500mg with no side effects. I am stage 4 CKD with high blood pressure. It’s contributed to a twenty percent reduction in BP but I’ve not yet taken my quarterly blood test per gfr, etc. so I’ll be curious to see if there are any improvements there.
Thanks for the lead on the niacin product as I’m also a stage 4 CKD with discussions underway with a transplant center. At this point in life of my late 60s, I have considerable questions about the associated “quality of life” issues surrounding kidney transplant. BTW, not a dialysis patient at this time so anything supporting that status continuing as long as possible is highly desirable.

Do note the eGFR formula forces the value down every year, even if your creatinine stayed the same.
However, if your protein or creatinine individually got worse, that’s a different matter and then this can be an option to get your prior numbers back.

Resveratrol is what I take daily. It helps control urine flow.
Healthy Origins Grape Seed Extract 300 mg (MegaNatural BP) Non-GMO California-Grown, 150 Vcaps
MegaNatural-BP is a specially-processed grape seed extract form that has had studies done on it. The one above, on a subscription from Amazon, seems the cheapest I can get it.
However, I more commonly get either the Zazzee or Bronson grape seed extract from Amazon, of which both are 400 mg. These have even more polyphenols, but some of these are not usable by the body, while the ones left in the MegaNatural-BP form are.
You probably get more of the “good stuff” in the MegaNatural-BP form, but it's cheaper to get the others, especially on a subscription.

you can buy timed release niacin. that’s the same niacin that flushes only its released in small quantities over time so you don’t notice it
