What if the thrusters had fired earlier, when it was near the space station but not docked? Maybe crashing into the station? I think the crew dodged a bullet.

Yea, when ever there is a screw up, always blame it on the software.

software can’t “fail”. It is a set of instructions that the hardware must follow. AI software might produce a solution that is not optimal, but it doesn’t fail. Ever. It might be a programming error, in which case the programmer and the testing and quality control organizations failed.

If I had to guess, it was a safety system meant to bring the module back into a good attitude (unlike Democrats) if it cycles. Somehow that got reset and thought the module was free-flying.
