Campaigner Noel Conway, who brought a legal challenge against the ban on assisted dying in the UK, has died at the age of 71.
Mr Conway, of Shropshire, had motor neurone disease and only had movement in his right hand, head and neck.
He took his campaign to the Supreme Court, arguing for his right to die, but lost the appeal in 2018.
His wife Carol said he had died at home on Wednesday after deciding with his family to remove his ventilator.
She said his carers "ensured Noel had a painless and dignified death, demonstrating empathy and concern for us all".
"Noel was in control, which was so important," Mrs Conway said.
"Ultimately, Noel wanted the choice of an assisted death and I hope his campaigning will bring this option closer to becoming a reality for other terminally ill people in this country."
He composed a statement in which he said his quality of life "has dipped into the negative".
He asked Dignity in Dying, which supported his legal action, to release his final words after his death.
"My voice has depleted to the extent that many people cannot now tell what I say and my eyesight recently deteriorated," he wrote.
"I'm already a paraplegic and I cannot use my hands or fingers but I am aware that my neck muscles are weakening as are my mouth and speech muscles.
"I recognise that the time has come to take the decision now to do something about this.
"I am not leaving it until I'm completely bed-ridden and unable to communicate at all."
He wrote how he felt he had "no alternative to ending my life without pain and suffering and without compromising others".
"This is not something I would have chosen."

given that he was pretty much totally paralyzed his options were rather limited.
He should have moved to Kanadahar. Prome Mistake True-dope brought in legislation allowing assisted suicide, for any reason, including depression.
Alas, he would likely not have been able to move here because he is of European ancestry and of Judeo-Christian heritage. If he were Asian, or African, the darker hued, the better, an atheist or mohamedan, and communist in outlook? True-dope would meet him at the airport and present him with the Order of Canada!

Mr. Conway found himself in a nightmare situation: already in a bad way, he could tell his ability to control his own fate was slipping out of his weakening grasp. I salute his courage, and am very sorry that he had to endure such a horrible illness and decline. He was the only one who could know when enough was enough. Everyone should have (does have in my view) the right to make that choice, and the law should recognize that. The challenge is preventing other people, government, and institutions from forcing their will over individuals.

Several surgeries, they wouldnāt give her an implant that matched the size of her remaining breast, insurance wouldnāt cover it, as one that size was by definition ācosmeticā not “restorativeā, (she was a big gal) forcing her spend the rest of her life asymmetric, radiation therapy, resulting in permanently scorched and non-healing tissue, four courses of really nasty chemo, she said āĀ”No mĆ”s!ā to a fifth course.
Her husband and I drove 500 miles to get her a puppy (she was delighted, a beautiful angelic smile). She passed within a couple days, the day after I had to leave.
She did it on her own terms
She was at peace, knowing she’d done all she could, and her sons were with her when her time came.
Tough lady, I doubt I could have gone through all she did.

I agree with you. Only he would know if he’d had enough of life given the cards he was dealt. There remains a great deal about the nervous system, about immunity disorders that we don’t know.
I am less quick these days to sit in judgment of someone else’s pain or quality of life.
Ah, so he died successfully without a doctor killing him.
